This is a Summary of Material Modification describing changes to your health plan adopted by the Board of Trustees. Please be sure that you and your family read this notice carefully and keep it with your benefit booklet for future reference. 

 At the recent Board of Trustees meeting, the Board approved the following Plan changes retroactive to January 1, 2023. Unless specified, these amendments apply to the Classic and Economy plans: 

Colonoscopies – Enhanced Benefits 

Effective October 1, 2022, all colonoscopy services provided by a professional Provider or facility will now be covered under the preventive benefit, regardless of preventive or diagnostic billing type. 

Mammograms – Enhanced Benefits 

Effective January 1, 2023, all mammogram services provided by a professional Provider or facility will now be covered under the preventive benefit, regardless of preventive or diagnostic billing type. 

Cologuard® 

Effective January 1, 2023, the Plan has been amended to provide coverage for a provider recommended COLOGUARD® screening as in-network under the preventive care benefit. Benefits will be subject to the Usual, Reasonable and Customary allowance. 

CPAP Devices 

CPAP devices have been previously covered by both Plans with an allowed frequency of one device every five(5) years. Beginning January 1, 2023, the covered frequency for CPAP devices will be twice every five(5) years. This benefit will continue to be subject to medical necessity and Plan cost shares.
